The Health Central Streamliners Team created a new culture of teamwork and communication among the ICU staff, including physicians, and standardized the central line insertion protocols. Strategies were applied through lessons learned from a Failure Mode & Effect Analysis and also in the participation in the Florida Comprehensive Unit-based Safety Program. New tools that were utilized include the central line bundle, checklist and cart with standardized supplies.

A reduction from 8.7 infections per 1000 central line days (Oct.2009) to 1.17 (Feb.2011) validates our success over a 17 month period. Health Central could now celebrate lower rates than Florida (currently 1.25) and the Nation (currently 1.46).

Health Central has demonstrated its commitment to improve the health of their patients and community. The journey is not over until a zero rate of central line infections has been achieved and maintained.

Lockheed Martin Missiles and Fire Control (LMMFC) organization developed a strategic vision for continuous improvements throughout all manufacturing processes to have the ability to respond quickly to our customer needs and drive toward flawless quality in our production. We can never lose sight of the fact that our products and services must operate flawlessly – our customers depend on it, in many cases with their lives. This was the guiding idea behind the strategic drive for systemic solutions leveraged across all programs to reduce defects (Supplier and In-house caused). Our ultimate objective is clear – protecting the Warfighter. Creating the necessary tools to support continuous improvements efforts aligns with this objective.

We recognized the need for a global system of well chosen metrics and tools to overcome the inefficiencies and variability in the existing individual platforms. Leveraging best practices from several of our leading Programs and utilizing the Lean Six Sigma (LSS) LM21 Path to Excellence, we developed the DPU (Defects per Unit) and Supplier Shop Floor Defects tools to form the common platform for MFC’s Executive Level Council (ELC) quality metrics. A cross-functional Quality team, together with MFC Quality Information Systems (QIS) developers, originated a set of tools that automated the Quality metrics and analysis part of the process, bringing unprecedented levels of efficiency and control to all MFC programs. The results have been tremendous; year over year double digit improvements for both ELC metrics (DPU and Supplier Shop Floor Defects). This amounts to achieving more than 56% reduction in Supplier Shop Floor Defects and more than 62% improvement in DPU which has been a strong factor in the continued success of our company.
